Prior to your initial appointment, it will be in your best interest to contact your insurance plan’s provider list to determine if I am an in network provider. You are also encouraged to verify with your insurance carrier your office co-pay per visit and if you have a deductible. It is the responsibility of the patient or guardian to pay any co-payments, and any other charges that are not covered by your insurance provider.
Should I happen to be an out of network provider for your insurance plan, I will be happy to provide you with a form you can send in to your insurance company for possible reinbursement. That means you will be responsible for the fees at the time of our appointment and, depending on your plan, a certain percentage of that may be paid to you by your insurance company.
I encourage you to call the number on the back of your insurance card to determine you "out-of-network" benefits are so there are not any scary financial surprises. Ask them where to send what's called a "Superbill", which is the form I will give you. I'll give you everything you need for that , you just need the address.
A twenty-four hour notice to the office is required if you are unavailable to attend your scheduled session. In the event this notice is not given, or a person neglects to show up for their appointment, they will be required to pay the full cost of the session. Insurance carriers cannot be invoiced for a missed appointment.
Cash, check and all credit cards accepted for payment. Payment is due at the beginning of the sesssion.
